Independent Physical & Occupational Therapy Evaluations

Comprehensive, in-person evaluations performed by licensed Physical Therapists (PTs) and Occupational Therapists (OTs), focused on:

  • Functional capacity and physical performance

  • Upper-extremity, hand, and musculoskeletal function

  • Activity tolerance and work-related abilities

  • Objective clinical findings relevant to legal questions

Evaluation Fee: $250

Medical Record Review & Case Consultation

Detailed review and analysis of:

  • Prior medical records and imaging reports

  • Therapy documentation and treatment history

  • Relevant functional or vocational information

Rate: $250/hour (billed in 0.25-hour increments)

Expert Opinion Report Preparation

Preparation of clear, defensible written expert reports, including:

  • Functional findings and clinical analysis

  • Professional opinions with supporting rationale

  • Summary of records reviewed and data considered

  • Description of evaluation methods and testing

  • Required disclosures, qualifications, and compensation statements

Reports may include:

  • Initial (pre-evaluation) reports

  • Post-evaluation or supplemental reports

  • Reports compliant with Federal Rules of Civil Procedure (Rule 26)

Report Fee: $1,500 per report

Deposition & Trial Testimony

Deposition Testimony

  • $500/hour

  • 2-hour minimum

  • Preparation billed separately

Trial Testimony

  • $700/hour

  • Half-day (4-hour) minimum

  • Preparation billed separately

Billing & Retainer Policy

  • $500 deposit required prior to scheduling the evaluation
    (covers evaluation + 1 hour of record review)

  • Report fees billed upon submission

  • Testimony fees due at least 24 hours in advance

  • All fees are non-contingent upon case outcome or opinions rendered

What are your fees?
Evaluation: $250. Records review: $250/hour. Expert report: $1,500 per report. Deposition: $500/hour (2-hour minimum). Trial: $700/hour (4-hour minimum).

Is a deposit required?
Yes. A $500 deposit is required prior to scheduling (evaluation + 1 hour record review). Testimony minimum fees are due at least 24 hours in advance.

Are fees contingent on the case outcome?
No. Fees are non-contingent upon case outcomes or opinions rendered.

Do you provide medical diagnoses?
