Rafael E. Salazar II, MHS, OTR/L
Occupational Therapist & Owner
Rafael E. Salazar II, MHS, OTR/L (Rafi) is a licensed Occupational Therapist based in Georgia. Rafi has worked in a variety of settings, from orthopedic and musculoskeletal rehabilitation, to academia, and even healthcare consulting. He spent the majority of his clinical experience working at Charlie Norwood VA Medical Center, where he was the lead clinician and clinical education coordinator for the outpatient specialty rehab program. In this role, he treated many veterans with chronic pain and helped to establish an interdisciplinary pain management program. He has worked on projects ranging from patient engagement initiatives to marketing communication campaigns to a multi million dollar project assisting the State of Georgia’s Department of Behavioral Health and Developmental Disabilities transition individuals out of state institutions to community residences. He has worked as an independent healthcare consultant since 2017, as the Principal Owner of Rehab U Practice Solutions, helping healthcare organizations and private PT/OT clinics develop effective patient engagement and retention strategies. Rafi also hosts The Better Outcomes Show, a podcast that explores the possibilities of a new healthcare. He has a passion for helping people overcome their limitations & pain to return to doing the things they love. He’s on a mission to make healthcare human again.
Janet Duggan, PT
An original founding partner of ProActive PT & Rehabilitation, Janet H. Duggan, PT received her physical therapy degree from Daeman College in New York. She has practiced physical therapy for 31+ years. Janet brings excellent leadership, organizational, and customer service skills to the practice. She has extensive experience managing an outpatient rehabilitation clinic, placing high value on quality of care and exceptional customer satisfaction. Her department consistently earned satisfaction scores of 97%! She holds an MBA from Auburn University. Janet has vast experience treating clients with neurological and orthopedic conditions in a variety of settings (inpatient rehabilitation, outpatient and acute care.) She completed the certification program for neuro-developmental treatment. She has a special interest in aquatic therapy techniques (including balance training).
Diane Wilson, PT, CEAS II
An original founding partner of ProActive PT & Rehabilitation, Diane Wilson, PT, CEAS II, a graduate of the Medical College of Georgia, brings 30+ years of patient-care experience to the practice. She has demonstrated successes in treating even the most complex orthopedic and neurologic conditions, including multiple-limb amputation, chronic pain, complex regional pain syndrome, cancer, migraine/headaches, myofascial pain syndrome, traumatic brain injury, balance disorders, and orthopedic problems in clients with neurological conditions. More than everything else, she enjoys the relationships that develop during one-on-one, hands on treatment sessions.
Courtney Nash O’Neal, DPT
Courtney graduated from Clemson University in 2010, and went on to get her Doctorate in Physical Therapy from Augusta University in 2013. She has treated children and adults since 2013, and enjoys working with patients on improving health and wellness, helping them meet their goals, and returning to their highest level of function.
Chris Rogers, PT
Chris graduated from The Medical College of Georgia (now Augusta University) in 1989 with a Bachelors of Science in Physical Therapy. He has over 32 years of experience delivering patient-centered care. He has worked in a wide variety of settings including outpatient rehab, inpatient rehab, skilled nursing, critical care, multi-trauma, and acute care. He is certified as a clinical instructor and in compression garment measurement.
Michelle Martin, PTA
Michelle graduated from Midlands Technical College with an Associate’s Degree in Physical Therapy Assistant in 2013. Prior to becoming a PTA, Michelles was a fitness instructor and worked as a personal trainer. She also holds a wellness certification in addition to her experience as a certified group fitness instructor. She has worked in outpatient rehab for most of her career as a PTA and enjoys helping her patients overcome their pain and return to their normal activities.
Tonya Cadwell, PTA, CMT, Licensed AFAA/NASM Trainer
Tonya has been a CMT for 27 years as well as a licensed PTA in the State of Georgia for 13. She received her bachelors degree in health scientific from Georgia Southern University in 1995, then continued her education & training in Sports Medicine. Tonya received her sports massage training in Atlanta and over the years, has worked in the areas of Sports, Orthopedics, and Inpatient/Acute Care facilities. In 2021, she received her NASM/AFAA Licensure in Fitness Training and intends to further her studies in the areas of Aquatics, Orthopedic Manual Treatment, and Nutritional Counseling.
Tonya’s mission is to integrate her studies into specific manual techniques that aid in the healing and recovery process following surgery, controlling pain, and improve function both in the water and on land.
