Packages & Options

We understand that thinking about doing any work to your house can cause stress and anxiety. We want to help make sure that the process of modifying or adapting your home to make it safer and more accessing is as smooth as possible. That's why we've developed these three package options. Each option includes a complete home assessment, report, and debriefing; so you can be sure that you're getting access the the recommendations of a trained and licensed therapist. The report includes a thorough analysis of the home, structures, and functional status of the person for whom the home assessment is being completed. It also includes detailed recommendations and options for modifying the building, adapting the environment, or using durable medical equipment.

Because of his previous work, Rafael E. Salazar II, MHS, OTR/L (Rafi) has numerous contacts in the home construction and modification industry. In fact, he developed the home assessment & modification forms, processes, and guidelines that Georgia's Department of behavioral Health & Developmental Disabilities (DBHDD) uses when transitioning individuals out of state hospitals to community residences.

How It Works

Alright, so without boring you to death, here’s the detailed process for our Home Accessibility & Safety Assessment & Consulting Services:

 

1. Functional Screening & Assessment

First thing’s first. We have to understand your individual needs and functional status before making any recommendations. To do that, we complete a comprehensive functional screening and assessment that includes areas such as: strength, pain, movement (range of motion), and functional mobility —that’s fancy talk for “how do you walk and get around”. We also assess any equipment that you may already be using for safety and accessibility, like wheelchairs, shower equipment, and the like.

 

2. Walk-Through

Next, we complete a thorough walk-through of your home with you. During the walk-through, we take measurements of doorways, hallways, bathrooms, bedrooms, kitchen, etc. We take pictures of the home for documentation, and to provide clarity when writing the final report. We do this walk-through with you and anyone else you’d like to be involved, so we can get a full and complete picture of the home, how you use it, and any problem areas that you’d like addressed.

 

3. Report & Debrief

After the walkthrough, we develop a comprehensive & individualized report that includes you or your loved one’s individual functional status and needs for safety & accessibility. We also include the objective measurements and pictures we got during the walk-through. This report also includes recommended modifications &/or equipment. These recommendations are tailored to you or your loved one.

After the report is finished, we schedule a debrief meeting with you, your family, or anyone else who you think needs to be included in the discussion. At this meeting, we will discuss the report and the recommendations that it contains. We’ll take time to answer any questions and to outline possible options for moving forward.

 

4. Follow-up & Consulting

We don’t just leave you hanging after the debrief meeting. We are available to provide consultation & guidance to you or any contractors or entities that may be completing home modifications or purchasing durable medical equipment. We can even be available to provide any final follow-up and training that may be helpful after modifications are complete and/or equipment has been purchased.

About Our Process

Rafi Salazar OT

Rafael E. Salazar II, MHS, OTR/L (Rafi) Owner, ProActive Rehabilitation & Wellness

Rafael E. Salazar II, MHS, OTR/L (Rafi) first began doing home assessments while working as a consultant on a multi-million dollar project for Georgia's Department of Behavioral Health & Developmental Disabilities (DBHDD). When he was working as a consultant for the department, one of his projects included finding a way to make sure that individuals who were leaving State Hospitals would be as safe and independent as possible in their homes once they discharged. This project involved not only making sure that the individual's accessibility and safety needs were well understood, but also that any modifications or equipment recommended was necessary and used properly. As part of this project, Rafi developed a Home Accessibility & Safety Assessment Process, which became a foundational piece of the transition process for individuals leaving State Hospitals.

After that project ended, Rafi then began offering this process to private individuals, workman's comp organizations, and other healthcare plans as a way to use this proven process and method to help anyone who may have accessibility or safety needs in the home.

Rafi has developed proprietary templates and resources for completing these home assessments and for guiding contractors and individuals through implementation of the recommendations.
