Rafael E. Salazar II, MHS, OTR/L
Occupational Therapist & Owner
Rafael E. Salazar II, MHS, OTR/L (Rafi) is a licensed Occupational Therapist based in Georgia. Rafi has worked in a variety of settings, from orthopedic and musculoskeletal rehabilitation, to academia, and even healthcare consulting. He spent the majority of his clinical experience working at Charlie Norwood VA Medical Center, where he was the lead clinician and clinical education coordinator for the outpatient specialty rehab program. In this role, he treated many veterans with chronic pain and helped to establish an interdisciplinary pain management program. He has worked on projects ranging from patient engagement initiatives to marketing communication campaigns to a multi million dollar project assisting the State of Georgia’s Department of Behavioral Health and Developmental Disabilities transition individuals out of state institutions to community residences. He has worked as an independent healthcare consultant since 2017, as the Principal Owner of Rehab U Practice Solutions, helping healthcare organizations and private PT/OT clinics develop effective patient engagement and retention strategies. Rafi also hosts The Better Outcomes Show, a podcast that explores the possibilities of a new healthcare and is the author of Better Outcomes: A Guide to Humanizing Healthcare. He has a passion for helping people overcome their limitations & pain to return to doing the things they love. He’s on a mission to make healthcare human again. Rafi also authored the book The Natural Runner: 8 Weeks to Pain-Free Running.
Courtney Nash O’Neal, DPT
Courtney graduated from Clemson University in 2010, and went on to get her Doctorate in Physical Therapy from Augusta University in 2013. She has treated children and adults since 2013, and enjoys working with patients on improving health and wellness, helping them meet their goals, and returning to their highest level of function. She’s also the author of You Can Do Hard Things: A Physical Therapist’s Guide to a Half Marathon.
Susannah Webb Azofeifa, DPT
Susannah was born and raised in Owensboro, Kentucky but now calls North Augusta, SC home. Susannah attended the University of Louisville and graduated with a bachelor’s degree in Kinesiology/Exercise Science in 2017. She then attended Breanu University where she received her doctorate of physical therapy in 2021. Susannah enjoys treating a wide range of orthopedic conditions and also specializes in treating pelvic floor dysfunction, and pre and post partum individuals. She enjoys spending time doing activities such as CrossFit, being outdoors, traveling, being with family, and spending time with her husband, Fabian. She is excited to join the ProActive team where she will continue to learn and grow both personally and professionally while helping patients optimize their overall function.
Kristen (Brooke) Suggs, PTA
Brooke attended the physical therapy assistant program at Orangeburg-Calhoun Technical College after studying exercise science at the University of South Carolina, Aiken. She has been practicing as a Physical Therapy Assistant since 2021 and is excited to join the team at ProActive Rehabilitation & Wellness.
Leah Klima, MHS, OTR/L
Leah Klima graduated from Auburn University (War Eagle!) in 2021 with a bachelors in Human development before graduating with her Masters from Augusta in 2024. Leah completed her rotation at Proactive where she found her love for the vast populations we serve. As an occupational therapist, she is dedicated to enhancing the lives of individuals through compassionate care and evidence-based practice. She brings a wealth of experience in promoting independence and improving quality of life. She enjoys spending free time with her family and friends traveling and spending time outdoors. Leah is excited to join the team at ProActive Rehabilitation & Wellness.
