Referrals & Prescriptions

Let’s tackle the big “referral” question. We get asked all the time: “Do I need a referral to get my knees (or shoulder, back, etc.) evaluated?”

The short answer is “No”.

In our state of Georgia, PT & OT have what is called “Direct Access”. This means that patients don’t legally need to obtain a referral from a physician to be evaluated and treated by a physical or occupational therapist.

There are, however some potential complications. For example, while the laws in Georgia do not require a referral for PT or OT, a person’s insurance plan may require a referral in order to cover those services.

We always tell patients that, if they’re concerned about their insurance coverage, we are able to look into that for them to make sure everything’s good to go before they come in for their appointment.

We also have referral forms available here, if you’re planning on seeing your physician in the office first. Just have them fax it over to us and we’ll get in contact with you.

Choosing a healthcare provider can be stressful, and we value the opportunity to develop a holistic plan to help you achieve your goals, overcome your limitation, and/or manage your pain.

Let’s start this relationship out on the right foot by providing you information that will help your first session go as smoothly as possible.

Scheduling

At ProActive Rehabilitation & Wellness, we understand that delays in treatment delivery can result in longer recovery times and sometimes poorer outcomes. We aim to schedule you as soon as possible after we receive a referral from your physician, or when you call to setup your first session. Typically, we are able to schedule new patients and clients within 1-2 business days. Most insurance providers require that you have a physician referral (also referred to as a consultation, prescription, or order —I know: why so many terms?) before they will pay for therapy services. If your physician sent the referral for your therapy directly to us, we will call you at the number he/she provided to schedule the appointment. If he/she gave the consultation request to you (or if you will be paying for your therapy out-of-pocket and therefore do not need one) please contact us and we will be happy to schedule your appointment.

Our clinic hours are Monday-Thursday: 8:00am – 5:00pm and Friday: 8:00am – 1:00pm

Your First Visit

Please wear comfortable, loose-fitting clothing that will allow you to move freely and your therapist to see and touch the affected body part. We don’t need any tight suites or high heels.

Arrive at least 15-20 minutes before your scheduled appointment time to register at the front desk. (We recommend you download registration forms to complete and email to us or bring with you, since you usually remember more details that way.)

If the client is unable to legally consent to treatment (e.g cognitive impairment) they must be accompanied by the person who is legally designated to make medical decisions, and we must have a copy of the legal document at the first visit. Any client under 18 years of age must be accompanied by a parent or legal guardian (legal document required to establish guardianship.)

Your clinician will perform a biopsychosocial assessment that may last last about 45 minutes. They will take a detailed medical and surgical history, review your current medications, and ask you about the specific problem that brings you to this appointment. They will then examine you, perform tests and take measurements.

Your clinician will take the time to listen to your story. We want to know how this injury, pain, or limitation is affecting your daily life.

Your clinician,  will discuss your treatment options with you and give you an opportunity to ask questions. Together, you will set goals and make a discharge plan. You may be given specific instructions or exercises to complete prior to your next visit. Before you leave, we ask that you stop at the front desk and set up your appointments.

You should plan to be at the clinic for an 60-90 minutes for the first session.

Things to bring to your first visit:

  • Picture ID
  • Insurance Cards
  • Prescription from your doctor (if he/she gave it to you)
  • Your appointment calendar
  • Any registration forms you completed at home but didn’t email to us. If you did not complete the medical history form at home, bring the following information to complete the form when you arrive.
    • A list of your medications with dosage
    • Medical history (all of your medical problems and diagnoses )
    • A list of surgeries you have had and the dates they were performed
    • A list of everything you are allergic to

Financial Policy

ProActive Rehabilitation & Wellness, promises to be a good steward of your financial resources. Heck, in our Manifesto, we commit to embrace transparency throughout the course of care, including the financial costs to you.

We accept Medicare, Tricare and most private insurance plans. Please contact us or your insurance company to make sure we participate with your insurance plan.

Unless you make other arrangements in advance, you will be required to pay any co-payments and deductibles at the time of service. We accept checks, Mastercard, Visa and Discover. We also work with clients that pay out of pocket to setup affordable payment plans.

Cancellations

We understand that unforeseen events arise and you may need to reschedule an appointment. Please provide at least 24 hours notice to allow your therapist to offer the appointment to another patient who may be in pain. You may be assessed a cancellation fee if you do not give 24 hours notice.
