Rafi

Rafael E. Salazar II, MHS, OTR/L

Occupational Therapist & Owner

Rafael E. Salazar II, MHS, OTR/L (Rafi) is a licensed Occupational Therapist based in Georgia. Rafi has worked in a variety of settings, from orthopedic and musculoskeletal rehabilitation, to academia, and even healthcare consulting. He spent the majority of his clinical experience working at Charlie Norwood VA Medical Center, where he was the lead clinician and clinical education coordinator for the outpatient specialty rehab program. In this role, he treated many veterans with chronic pain and helped to establish an interdisciplinary pain management program. He has worked on projects ranging from patient engagement initiatives to marketing communication campaigns to a multi million dollar project assisting the State of Georgia’s Department of Behavioral Health and Developmental Disabilities transition individuals out of state institutions to community residences. He has worked as an independent healthcare consultant since 2017, as the Principal Owner of Rehab U Practice Solutions, helping healthcare organizations and private PT/OT clinics develop effective patient engagement and retention strategies. Rafi also hosts The Better Outcomes Show, a podcast that explores the possibilities of a new healthcare and is the author of Better Outcomes: A Guide to Humanizing Healthcare. He has a passion for helping people overcome their limitations & pain to return to doing the things they love. He’s on a mission to make healthcare human again. Rafi also authored the book The Natural Runner: 8 Weeks to Pain-Free Running

Courtney

Courtney Nash O’Neal, DPT

Courtney graduated from Clemson University in 2010, and went on to get her Doctorate in Physical Therapy from Augusta University in 2013. She has treated children and adults since 2013, and enjoys working with patients on improving health and wellness, helping them meet their goals, and returning to their highest level of function. She’s also the author of You Can Do Hard Things: A Physical Therapist’s Guide to a Half Marathon

Chandler

Chandler Pate Dittmer, MHS, OTR/L

Chandler Pate Dittmer graduated from Augusta University with a BS in Kinesiology, and then stayed and went on to get her Masters in Occupational Therapy in 2021. Chandler completed a rotation at Doctor’s Hospital outpatient hand clinic where she discovered her passion for serving patients with orthopedic and neurological injuries.  She then completed a fieldwork rotation at the Charlie Norwood Veterans hospital in the Spinal cord unit, where she learned how to deliver client centered care with skill and compassion. Chandler is excited to be joining the team at ProActive and is looking forward to learning from, and serving each and every patient.

Susannah

Susannah Webb Azofeifa, DPT

Susannah was born and raised in Owensboro, Kentucky but now calls North Augusta, SC home. Susannah attended the University of Louisville and graduated with a bachelor’s degree in Kinesiology/Exercise Science in 2017. She then attended Breanu University where she received her doctorate of physical therapy in 2021. Susannah enjoys treating a wide range of orthopedic conditions and also specializes in treating pelvic floor dysfunction, and pre and post partum individuals. She enjoys spending time doing activities such as CrossFit, being outdoors, traveling, being with family, and spending time with her husband, Fabian. She is excited to join the ProActive team where she will continue to learn and grow both personally and professionally while helping patients optimize their overall function.

Kristen (Brooke) Suggs, PTA

Brooke attended the physical therapy assistant program at Orangeburg-Calhoun Technical College after studying exercise science at the University of South Carolina, Aiken. She has been practicing as a Physical Therapy Assistant since 2021 and is excited to join the team at ProActive Rehabilitation & Wellness.

Debora Corley, MPT

Deborah is a physical therapist who joined ProActive in the Spring of 2022. She received her Masters of Physical Therapy from The Medical College of Georgia in 2002. She has worked in a variety of settings including skilled nursing, assisted living, home health, acute care, and outpatient therapy. Her drive comes from a commitment to helping patients manage their pain and injuries.
