Our Philosophy of Care for Providing PT & OT Services

 

ProActive’s Rehabilitation Program Framework

 

As we know from the literature and available research, conditions that require the treatment of physical or occupational therapy rarely resolve with infrequent visits or carryover in home programs. For example, the minimum dose to protect older adults from falls is 50 hours [1]. This likely requires more than a single visit, or even higher frequency than 1 visit every other week or even once per week. 

 

In fact, research also suggests that patients experiencing musculoskeletal pain, including chronic pain, show greater improvement and adherence to home programs by following 8 weeks of supervised physical therapy exercises, twice per week [2].

 

That being said, at ProActive Rehabilitation & Wellness, we take a IV Phase approach to Rehabilitation Programs and our PT & OT Services.

 

4-Phase Rehab Programs at ProActive

 

ProActive Rehabilitation & Wellness adopts a 4-phase rehabilitation approach to treatment and delivery of our PT & OT Services. These four phases may overlap and not necessarily be distinct, but it helps to think about your plan of care as having these 4 phases. 

 

Below are the 4 phases of ProActive’s rehab program development, specifically for musculoskeletal pain (MSK) and orthopedic conditions:

  • Phase I: Pain Relief
  • Phase II: Retraining & Return to Normal Movement 
  • Phase III: Functional Performance and Targeted Improvement (Strengthening) 
  • Phase IV: Self-Management & Discharge with Independence 

 

How often should I attend PT & OT treatments?

 

Research supports increased frequency in physical & occupational therapy treatments, over a set & defined course of treatment as a way to improve clinical outcomes and functional improvement [4]. In fact, research also suggests that the optimal frequency for PT & OT Services for orthopedic & musculoskeletal pain/dysfunction approaches 2+ visits per week [4].

Our goal at ProActive Rehabilitation & Wellness is to empower our patients to overcome their limitations and return to the activities and roles which they value the most, and to do it in the most effective & efficient way possible. That means we take research like this into account when we develop your plan of care and deliver PT & OT Services.

At least during the initial phases of a rehab plan of care, we recommend planning for at least 2 visits per week. As you progress through the program, we taper down the frequency of those visits until you eventually discharge (or graduate) from your rehabilitation program.

Our goal is to help you overcome their pain  and return to normal as quickly as possible. That requires frequent visits, at least initially, for a defined period of time. You can’t simply “stretch” it out a few extra weeks by dropping down to 1x/wk. As the research indicates, you cut your odds of success by almost 96% when you decrease frequency in a plan of care.
